It takes a community to prevent a fall

Overview

"I don’t want to bother my GP” are words from older individuals who have suffered a fall. There are currently no easy to access, appealing, solutions for this problem. We are presenting a novel pathway to allow older adults to self-assess their risk of falls and fractures and offer easy access to the healthcare system. 
 
We have created a computer-based questionnaire which older adults can access in community locations and online. This calculates their risk of having a fall or fracture. Those identified as “at risk” will be invited for a full assessment and subsequently referred to relevant healthcare services, prior to any problems developing and/or worsening. All users will be given access to a freely available educational resource pack on fall and fracture prevention.

This proposal is an alternative way for older adults to access the healthcare system in future, empowering them to lead long, healthy lives.
