Issa Dasu Patel, CEO & Cofounder at CONNECT Care
DeeDee Wallace, Health & Care Innovation Lead at Oxfordshire County Council
Sally Steele, Home First System Lead, NHS Buckinghamshire, Oxfordshire, & Berkshire West ICB

For too long, we have lived in a world where health & care is done to us rather than with us. That’s finally changing, catching up with people’s expectations. Despite medicines being the most common intervention in healthcare, they’ve lagged behind in this transformation, at the cost of people’s independence. This panel explores a high profile project being carried out in Oxfordshire at the intersection of health, social care, and pharmacy to radically rethink the medication support pathways. Helping people in the community as they age: positively, actively, independently, and with dignity.

Everyone has encountered frustrating and repetitive data collection. Even more so if you want to delegate responsibilities to family, friends and professional carers. This facilitated workshop session paints a picture of how person-centric data could work and will help delegates consider the design of the digital future you.
